Changing your contact details

To update details like your name, address, phone numbers or email address:

  • use your NHS account (through the NHS website or NHS App)
  • use the “Contact Us” option in the green banner on our website landing page

Change of address

If you have moved to a new address, and this is not in our catchment area, we will need to review your request to ensure it is appropriate for you to remain registered with us.

Changing your phone number or email address

You will need to show us identification before we can change your phone number or email address.

Changing your name

You will need to show us proof that you have changed your name. This could be a marriage or civil partnership certificate, deed poll or birth certificate.
