Data Provision Notice

Sharing information with NHS England for planning and research

Under the Health and Social Care Act 2012, NHS England (formerly NHS Digital) has the legal authority, through Section 259(1), to issue a Data Provision Notice (DPN). This notice requires healthcare organisations, including GP practices, to share specific patient data for purposes such as health service planning and improvement. However, you have the right to opt out of your confidential patient information being used for purposes beyond your individual care by registering a National Data Opt-Out. This ensures your data is not included in such collections, unless there is a legal obligation to do so

To opt out please complete the opt out form and return it to the practice by email or hard copy.
