Sharing information with NHS England for planning and research
Under the Health and Social Care Act 2012, NHS England has the legal authority to issue Data Provision Notices (DPNs), requiring healthcare organisations, including GP practices, to provide specified information for purposes such as planning, commissioning, research and improving health and care services.
Patients have choices about how their confidential patient information is used for research and planning. These include a Type 1 Opt-Out, which prevents GP practices from sharing GP data for research and planning, and the National Data Opt-Out, which allows patients to opt out of NHS England and other health and care organisations sharing their confidential patient information for research and planning, except where there is a legal obligation or other exemption.
For a Type 1 Opt-Out, please complete the attached form and return it to the practice. Information about the National Data Opt-Out is available on the NHS website.
